Adults born under UK sugar rationing have up to 69% less cancer decades later
A UK Biobank study of 64,000+ adults found early-life sugar rationing cut cancer risk up to 69%, plus lower heart disease and dementia risk, decades later.

Every person in this story ate the same wartime diet as an adult. The only thing that differed was whether they were still drinking milk from a bottle when Britain's sugar ration ended.
That accident of birth date is now one of the cleanest natural experiments in nutrition science, and a UK Biobank research team has spent the last two years mining it. The headline result, published this year: adults whose first 1,000 days of life fell under sugar rationing have up to 69% lower rates of several cancers, decades later, than adults born just a few years after them.
The experiment nobody designed on purpose
Britain rationed sugar from 1942 to September 1953. Sugar consumption nearly doubled almost overnight once the ration lifted — it was the only food category that spiked that sharply. Children under two received no sugar allocation and no sweets at all.
That created two groups of babies, born only months apart, who were alike in almost every other way: same country, same postwar economy, same healthcare system, same genetics on average. The only real difference was whether their first 1,000 days — conception through age two, the period nutrition researchers treat as the highest-leverage window in human development — happened before or after the ration ended.
Economists Tadeja Gracner, Claire Boone and Paul Gertler used that split as a natural experiment, comparing UK Biobank participants born right around the 1953 cutoff. They are not the only recent team to mine this cohort, but they are the ones who started it, and their group has now published a family of follow-up papers using the same birth-date discontinuity to check different diseases against it.
What they found, disease by disease
Cancer. The most recent installment, published in PNAS, followed more than 64,000 UK Biobank participants born between 1951 and 1956. Adults with the longest rationing exposure in early life had markedly lower rates of five cancers decades later: liver cancer risk was about 69% lower, prostate 52% lower, lung 41% lower, rectal 40% lower, and breast cancer 36% lower. Early-onset cancer risk overall dropped by roughly a third.
Diabetes and hypertension. The original 2024 paper in Science (summarized by USC Dornsife, DOI 10.1126/science.adn5421) found that early-life rationing reduced type 2 diabetes risk by about 35% and hypertension risk by about 20%, and pushed back the age of onset by roughly 4 and 2 years respectively. Protection showed up even with in-utero-only exposure and got stronger the longer rationing continued after birth, especially past 6 months — the point where most babies start eating solid food.
Heart disease. A follow-up study on 63,433 of the same participants found that people exposed to rationing in utero plus their first one to two years had a 20% lower rate of cardiovascular disease overall, 25% fewer heart attacks, 26% less heart failure, 24% less atrial fibrillation, 31% fewer strokes, and 27% lower cardiovascular mortality. Disease onset was delayed by an average of about 2.5 years. Diabetes and hypertension together explained roughly a third of that protective effect — meaning most of it came from somewhere else.
Dementia. A separate analysis of 64,737 of the same participants, published in Neurology and summarized by the American Academy of Neurology, found sugar rationing exposure was tied to a 21% lower risk of all-cause dementia (in utero plus the first year) rising to 23% lower with exposure through age two, with dementia onset delayed by an average of 2.6 years.
Biological aging. The cancer paper's researchers also measured telomere length — a marker of cellular aging — in the rationed cohort and found it was measurably longer, equivalent to roughly 2.2 years of slower biological aging.
The part that should change how you read this
The obvious objection: maybe rationed kids just kept eating less sugar their whole lives, and that's the real explanation, not some fixed early-life effect.
The researchers checked, and the honest answer is: partly, yes — and that itself is the finding. Around age 50, decades after rationing ended and long after everyone had equal access to sugar, people who were rationed as babies were still eating less of it and eating smaller portions with more varied diets than people born just after the cutoff. A restriction imposed for the first two years of life appears to have shifted taste preferences and eating habits that persisted for 50 years. The early window did not just protect the body temporarily — it seems to have set a pattern that stuck.
None of this means sugar exposure after age two is harmless, or that these effect sizes would replicate exactly in a different country's food system today. It is one cohort, one country, one historical accident, and hazard ratios in an observational natural experiment — even a well-designed one — are not the same as a randomized trial. But a natural experiment this clean, checked against five separate disease categories by more than one research group, using the same objectively defined birth-date cutoff each time, is about as strong as nutrition epidemiology gets without literally rationing sugar for a new generation of infants.
What US guidance already says, and how far off practice is
The Dietary Guidelines for Americans have recommended, since 2020, that infants and children under age two avoid all foods and drinks with added sugar entirely. That guidance already reflects the same first-1,000-days logic the UK data now backs up with hard outcomes.
Current US feeding practice is not close to that standard. A WIC Infant and Toddler Feeding Practices Study of 3,835 low-income children found that about 25% — roughly one in four — had already been given added sugar by 7 months of age.
Packaged food is a large part of why. A 2026 study in the journal Nutrients examined 651 baby and toddler products across the top 10 US grocery chains and found 71% of baby foods were ultra-processed. Ultra-processed products contained close to double the sugar of non-ultra-processed ones — 14.0g per 100g versus 7.3g — and added sugars showed up exclusively in the ultra-processed group. Snack-size packages were the worst offenders: 94% of them were ultra-processed, against 86% of full-size packages and 73% of pouches. A separate audit of nearly 2,800 toddler products found an even higher rate — 81% ultra-processed — with many failing WHO nutrient standards on sugar or sodium.
How to shop this
- Under age two, added sugar is not a "moderation" ingredient — US guidelines say skip it entirely. That includes fruit juice concentrate, cane syrup and other added-sugar aliases on baby food labels, not just the word "sugar."
- Pouches and full-size jars are not automatically safer than snack packs. All three categories ran ultra-processed in the majority in the 2026 audit; snack-size was worst, but pouches were not clean.
- Read the ingredient list, not the front-of-package claims. "No sugar added" is a real, regulated claim worth looking for; "natural," "wholesome" and cartoon fruit on the label are not.
- The first 1,000 days are the highest-leverage window you get. This data suggests habits and protection built in that window can outlast the window by decades — which cuts the other way too: what a baby eats now may be shaping preferences for a very long time.
